У меня странная ситуация. Примерно через три года на сервере с низким трафиком один из двух дисков Samsung в RAID1 вчера вышел из строя:

Поскольку smartd ничего не сообщил, я проверил интеллектуальные атрибуты, и единственное подозрительное чтение, которое отличается от sdb (сбой) по сравнению с sda:

Здесь возможны ошибки контроллера. Прежде чем вы потратите на это слишком много времени, я бы посоветовал переустановить все кабели. Возможно, что-то со временем вышло из строя и вызывает проблемы.
Кроме того, вы упомянули smartd ... вы отключили это при попытке запустить самотестирование? Возможно, это мешает ручным тестам.
Было ли что-нибудь в dmesg
о том, почему sdb считается неудачным? Похоже, что оба диска сообщают о своем состоянии как ПРОШЛО, и я не верю, что mdadm действительно использует какие-либо данные SMART для определения состояния диска.
