Загрузчик загрузил ядро в память и распаковал файл initrd. Процесс init запущен и попытался смонтировать постоянное хранилище (то есть: корневую файловую систему), после чего initramfs отключен, и загрузка продолжится из постоянного хранилища.
Ваш initrd не может смонтировать корневую файловую систему, поэтому init умирает, следовательно, ваша паника ядра.
Я бы попробовал разделить / и / boot на отдельные разделы и использовать ext3. Я мало что знаю об Amazon или VirtualBox, но знаю, что Red Hat не будет поддерживать корневые файловые системы ext4 на EL5, поэтому это не предполагаемый или проверенный вариант использования.
